A dermatophyte infection. Ringworm is the most common fungal presentation here, and heat, humidity and fitted clothing all make it worse — which is why it clusters in the hot months and under waistbands, in the groin and between toes.
1. How to recognise it
A ring with a raised, scaly, actively itchy edge and a clearer centre. It expands outward over days.
2. Use an antifungal, not a steroid
Terbinafine or an azole cream. A steroid cream calms the itch for a day and then lets the fungus spread — this combination is the single most common reason a case becomes extensive here.
3. Beware combination creams
Many popular over-the-counter tubes mix an antifungal with a strong steroid. They feel effective and reliably make the infection worse over weeks.
4. Treat past the point it looks better
Continue for one to two weeks after the rash disappears, or it returns.
5. Treat the environment too
Wash and hot-dry towels, bedding and clothes. Do not share them. Keep the area dry — fungus needs moisture.
When it needs a doctor
Scalp involvement, nails, a large area, recurrence, or anything in a child. Scalp ringworm in particular does not respond to cream and needs oral treatment.
